Name for certificate section: ie: "The Smith Family"   "Mr. & Mrs. Smith"     Greeting for Miracle Story section: ie: "Dear Smith Family,"  "Dear Mr. & Mrs. Smith,"     Your First Name:     Your Last Name:     Your FULL Office Name:     Your Email Address:     Your Office's Area Code: this determines which honor card is generated (201), (732), (908), or (973) (609) or (856)     PLEASE NOTE: In order to properly format the card, limits are placed on the length of Certificate Names and Greetings. If your certificate name or greeting does not fit in the space provided, try rephrasing it. For example if "The Smith Family" does not fit, try shortening it "The Smiths" instead.
